AUDIENCE: |
Case Managers, Community Care Assessors, Service Managers and Coordinators |
PRESENTER: | Glen Sorensen – Managing Director of Age Communications |
LOCATION: | Online – Zoom |
This 90-minute session shows how information gathered in referral, intake and assessment processes can be used in combination with basic motivational interviewing techniques to co-design client-set goals aimed at improving function and well being and maximising independence.
It also builds skills in how to frame goals in support plans so that they are focused on what the client wants to achieve or be different rather than just the tasks involved in getting there.
Past participants of Getting to Client Set Goals have reported:
- Thorough, clear and easy to understand. Great delivery throughout.
- I have been working as an assessor in the community sector and I have learnt more today than I’ve taken the time to learn in over 25 years.
- Very interactive, relevant and engaging workshop.
